The Aged Care Assessment Team (ACAT) assesses the needs of frail older people and helps arrange access to available care services appropriate to their needs.
ACAT determines eligibility for Commonwealth subsidised Aged Care Services, such as Community Home Care Packages and Residential Aged Care Services. The target group are people with an aged related condition.

Women’s Health Matters provides advice, counselling and treatment for menopause, sexual health, emotional wellbeing, urinary continence and osteoporosis to women aged 40 and over.
In some cases, we also provide care to women under the age of 40 who may have specific women’s health concerns.
Appointments with the Women’s Health Doctor or Counsellor can be made by phoning reception Monday to Friday.

9.00am - 5.00pm Mondays, Tuesdays and Wednesdays.

Our social work service supports clients in the Lismore local government area with a range of issues, including accessing or navigating community support services, managing health while experiencing homelessness, personal and family issues and coping after a traumatic event,
In some cases, this may also include psycho-social assessment and referral, counselling or advocacy.
We accept referrals from hospitals, GPs, specialists, allied health professionals, non-government organisations, family and carers, and self-referral.
Our Podiatrists work with clients who have foot and leg disorders to improve their mobility and enhance independence. This is achieved through the diagnosis, treatment, prevention, and management of pathological foot problems and associated restrictions in function.
The Podiatry service provides ulcer debridement and offloading, diabetes foot assessments, orthotic treatment, and help with applications for medical grade footwear.
The Palliative Care service supports patients with a progressive, life-limiting illness, to help maintain the best possible quality of life.
Specialist Palliative Care Nurses and our multi-disciplinary team including social workers, help patients monitor pain symptoms and offer support for end of life care in collaboration with the patient’s General Practitioner and treating specialists.
Our community occupational therapist provides home assessment and intervention with the aim of helping people stay as independent as possible, and safe within their own homes.
Our services include home modifications, equipment prescription, and education about falls prevention, energy conservation, and pressure injury prevention and management.
We accept referrals from GPs, allied health professionals, community service providers, family and friends with the client’s consent, and self-referral.
The Knee and Hip Arthritis Service aims to assist patients with Osteoarthritis of the hip and/or knee to manage their symptoms during all stages of the condition.
Patients will receive a comprehensive assessment and management plan looking at all aspects of health, not just joint pain.
We accept referrals from General Practitioners, Orthopaedic specialists, Allied Health and patients on NNSWLHD waiting lists for joint replacement surgery.
To access the service, a patient requires confirmed X-RAY evidence of Osteoarthritis in the hip or knee.
Hospital in the Home allows eligible patients to access hospital services, such as nursing and medical care, in the comfort of their own home.
Our Hospital in the Home team works with patients, families and carers to providing ongoing medical treatment that would otherwise be provided within the hospital. This includes physiotherapy services including respiratory and much more.
Patients are classified as inpatients of Lismore Base Hospital and cared for in collaboration with treating specialists and consultants.
